Lakeside House is a non-profit, New York State Office of Mental Health, certified 24-hr. supervised community residence program, serving ten (10) adult individuals, 18 years of age or older. Our transitional homelike setting offers attention to individual needs. Our program offers treatment and goal - oriented services in a safe, supportive, residential environment. We serve to enhance the quality of life for persons affected by mental illness, by working with all segments of various communities to ensure understanding, acceptance and responsiveness to the needs of people facing mental illness.
We believe that our mission is best served by empowering our residents, staff, volunteers and the community as equal participants in planning, development and implementation of services. We believe that awareness and integration of everyone in the community is essential to enhancing the quality of all of our lives. We also believe that a not-for-profit entity has an inherent responsibility to itself and its community to be creative yet conservative in managing its financial concerns. This position provides the agency with opportunities to continue improvement of the quality of its services and ultimately the lives of our residents. With care and support from knowledgeable, caring professionals and volunteers, individuals affected by mental illness can learn to cope with life's challenges, work meaningful jobs, and participate in all that life has to offer. We believe that empowering individuals is an essential process in successfully enhancing the quality of life.
Admissions to Lakeside House are made from many sources. An average of 10% of our referrals come from psychiatric or medical centers, 30% of our referrals come from Article 28 facility discharges, and 60% of our referrals come from non-inpatient community sources, like physicians, mental health clinics, and substance abuse programs. These individuals may be transitioning from psychiatric hospitals to more independent living environments or to those individuals whose hospitalization may be prevented by admission to our program.
Formal annual affiliation agreements exist between St. Lawrence Psychiatric Center, Lakeside House, and Franklin County Community Services. The major provisions of this agreement include referral and admission processes, which include individual and joint agency responsibilities; admission procedures; general admission requirements; oversight and corrective action processes.
Most of residents have co-existing secondary diagnoses, which include but are not limited to alcohol, substance abuse histories, and personality disorders. Increased emphasis is placed on a responsiveness to the various needs and desires of the individual in residence and on interventions that target any individual's specific functional and behavioral deficits in order for them to access and succeed in a living environment of their choice.

Our program promotes psychiatric rehabilitation, responding to the desires, tolerance and capacity to participate in such interventions for the purpose of reaching their community living, social and work oriented goals. All of the Part 595 required eleven (11) restorative services are offered to our residents. They are symptom management, medication management, health services, assertiveness / self-advocacy training, daily living skills training, skill development, community integration / resource development, socialization, parenting training and vocational and employment opportunities. We also provide case management services, computer skills training and transportation.
Each resident's individualized service plan will be reviewed at least once every three months, with the initial review occurring three months from the date of admission. The review will include participation of the resident, staff and resident collaterals. Such review will focus on objective attainment and will include assessment of progress, a sensitivity toward the individual's strengths and needs, a review of any obstacles hindering progress, and any recommendations for adjustment of goals, objectives, time frames for achievement, intervention strategies or the initiation of the discharge plan. Discharge goals and plans will be begin at admission and will be reviewed with respect to the status of the resident's progress, objective attainment and resident concurrence.
The anticipated length of stay and service plan goals will be presented to all applicants to the program as part of the orientation, admission and discharge planning process. It is anticipated that young applicants will be able to move to appropriate, less structured and more independent living environments within 3 to 36 months. Anticipated length of stay for elderly or medically frail residents may be longer. Goals toward more independent or less restrictive living environments may not be appropriate for these residents. From our program they may be headed towards a skilled nursing facility or a nursing home. Time frames for movement is not rigid, and flexibility can occur based on resident's needs, capacity and tolerance to progress toward their goals |
